The Lacey Jarrell Foundation Application For Educational Scholarship
Please complete the form below and submit the application when finished. In addition to the form, you are required to present an additional three things:

Academic Records
Please submit a copy of your current academic records. This must include a copy of your most recent report card and verification of current enrollment in an educational institution.
Letter of Recommendation
Please provide a minimum of one letter of recommendation from someone other than a family member who is over 21 years of age.

Written Essay
Please attach a written essay with a minimum of 500 words. The essay should explain why you desire to continue your education. Why are you motivated to further your education? It is not required that your response be relevant to Art, as scholarships are provided for all areas of interest. Please include a personal story that will help the selection committee better understand your motivation. Please also include any stories or comments on how you view your current progress from a child into adolescence or young adulthood. What have been your strengths? What have been your weaknesses? If you have experienced challenges in your life, how have you coped with these?
The Lacey Jarrell Foundation believes that all young people deserve a chance and therefore consider a student’s passion and desire for a better future as most important when selecting eligibility for a scholarship. We do not judge an applicant simply by his or her grades or GPA. Grades are important but we also understand that grades do not solely define who a student is or who a student can become.
We are also happy to support you as you work through the application process by answering questions, giving suggestions, or assisting you in any way we can. Please don’t hesitate to ask us for help if needed. It takes courage to ask for help and is a sign of strength. We want you to be successful!
